  • Alimony (Maintain copy of post - April 30, 1977 agreement or election, plus amendments)

  • Child care expenses (receipts) - For individual providers, include SIN and address - For summer camps, indicate number of weeks in residence
  • Attendants care expenses (receipts)
  • Allowable Business Investment Loss (ABIL)
  • Moving expenses (receipts). Distance moved to new employment
  • Alimony or separation allowances paid to an ex-spouse (Provide name and address of recipient(s). Maintain copy of agreement or court order for spousal support signed on or after May 1, 1997)
  • Commission and employment expenses (Provide details and T2200 or TL2)
  • Carrying charges (Interest on money borrowed to earn dividend and interest, investment counsel fees, interest for limited partnerships and safety deposit box)
  • Other deductions and expenses (receipts)
  • Federal and provincial political contributions (receipts)
  • Charitable donations (receipts)
  • Medical expenses (receipts). (Provide details of private health insurance premiums, including travel health insurance.)
  • Disability deduction for yourself or dependant (Obtain T2201 signed by physician if this is a first-time claim)
  • Tuition fees - T2202/T2202A (Include amounts that can be transferred from dependants)
  • Labour-sponsored funds - T5006
  • Interest paid on student loans (Obtain reporting slip)
  • Wholly-dependant persons (For each dependant provide name, address (if different from yours), relationship, birth date, SIN and net income. If applicable, note infirmity)

  • 2008 Installments (Attach February 2009 notice.)
  • Copy of T1-2008 assessment notices
  • Details of RRSP - Home Buyers Plan or Lifelong Learning Plan, withdrawals and/or the CCRA Statement of Account, if received
